Measles, Mumps, Rubella

Students must provide an official record of their measles, mumps, and rubella immunity before registering for classes. Records may be obtained from the student’s past or current health care provider, previous school or university, public health department or government agency. Students may utilize UB’s Health Background Form for compliance. The following information describes the proof of immunity required for measles, mumps, and rubella:

• TWO doses of Measles vaccine (or two doses of combined MMR vaccine) given on or after the first birthday, and at least 28 days apart. Vaccines given prior to 1968 are only valid if documented to be live vaccine without gamma globulin, or
• Physician documented history of the disease; or
• Serologic evidence of immunity (positive blood titers); or
• Proof of attendance in a U.S. primary or secondary school after 1980, PLUS proof of a second dose of live measles virus vaccine administered within one year prior to attending UB.

• One dose of Mumps vaccine (or combined MMR vaccine) on or after the first birthday, given after January 1, 1969, or
• Physician documented history of disease; or
• Serologic evidence of immunity (positive blood titers)

• One dose of Rubella vaccine (or combined MMR vaccine) on or after the first birthday, given after January 1, 1969, or
• Serologic evidence of immunity (positive blood titers). (Note: History of illness is not acceptable).

We will accept any one of the following documentation of your MMR vaccinations:

• A copy of the Health Background Form (PDF (160KB) | Word (75KB)) completed by the student and their health care provider including a copy of your immunizations (including actual dates); OR
• A copy of your immunizations (including actual dates) on an official government/ school letterhead — the simplest place to obtain this may be from your most recently attended high school or college; OR
• A copy of your immunizations (including actual dates) on physician’s letterhead, which includes printed name, address and telephone number; OR
• Have a blood test to confirm immunity. Please note: a copy of the lab report must be provided to Student Health Services.

Please note that the documentation must be in English and must have students name and date of birth.

The MMR vaccination is available at Student Health Services. Please call 716-829-3316 for more information.

• Students born prior to 1/1/1957 are exempt.
• Distance-learning students who do not meet on campus are exempt.
• Medical reasons:
  • For a temporary or permanent medical exemption, the student must submit a signed statement from a health care provider that includes the reason, specific immunizations that apply, and an end date for the exemption.
• Religious reasons:
  • For a religious exemption, the student must submit documentation of a sincerely held religious belief. Statements from parents/guardians will only be accepted if the student is less than 18 years of age.
• Active military or honorably discharged:
  • For military exemption, the student in active military status must provide a copy of their military ID
  • If discharged, the student must provide a copy of their DD 214 showing their honorable discharge

Note: Students with medical, religious, or military exception will be allowed to register for & attend classes: however, those students who are not documented to UB as immune may be excluded from campus in the event of a measles, mumps or rubella outbreak, per New York State Department of Health policies.

Meningitis Information

Students must verify that they have received information about meningococcal disease and have made an informed decision about whether or not to receive vaccination against meningococcal disease. Students comply by reading the required information regarding meningitis, and completing the meningococcal information response form. The student is considered compliant with the requirement whether they received the vaccination or declined it.

Meningitis Information Requirement

The Meningitis Information Response Form is located on HUB. Go to www.myub.buffalo.edu and log in with your UB IT name & password. Then click on the HUB Student Center tab in the MyUB header.

If you have the MEN hold, a window will pop-up; complete the form per the instructions on the pop up. The MEN hold will disappear in real time if submission was successful.

If you don’t have the MEN hold, but want to comply thru HUB Student Center, from your Main Menu, select University of Buffalo, select Immunization Waiver. Complete the form per the instructions on the screen.

Health-Related Profession Students

In addition to the Measles, Mumps, Rubella (MMR) and Meningitis Information requirements, Health-Related Profession students must provide documentation of:

Hepatitis B vaccine series – All students must begin the vaccine series prior to starting their academic program and must be completed by the end of the program's first year.

Tetanus/Diphteria/Pertussis - All students must have received a booster within the last 10 years.

Chickenpox (Varicella) Immunity – All students must demonstrate immunity through one of the following:

• Two doses of Varicella vaccine (time period between doses varies by age of recipient); OR
• Documented history of disease; OR
• Serologic evidence of immunity (positive blood titers).

Tuberculin Skin Test (PPD) - All students with no prior history of positive PPD should have a Mantoux Tuberculin Skin Test, QuantiFERON-TB Gold or T-SPOT within 12 months prior to starting their program. (Note: Tine Test is not accepted; Prior BCG or pregnancy is not an exemption; Chest X-Ray cannot be substituted for PPD; Chest X-Ray is required if PPD is positive or if student has a prior history of a positive PPD).

For each year of their academic program, Health Related Profession students must complete an immunization update – including a PPD test (or symptom review for previous positive PPD). These requirements can be fulfilled at Student Health Services. Call 716-829-3316 to schedule an appointment. If you choose to complete the requirement outside of Student Health Services you may use the Annual Immunization Review form for compliance. Please return the form to Student Health Services once completed. Official due dates will be determined and announced by the individual departments.
